What is collected and why
The form collects your name and contact details to reply and arrange a meeting. It also records the enquiry date, processing status, the policy version you agreed to, and notification delivery status. Please do not send medical documents, personal histories or personal data about a child.
Providing data is voluntary. The form cannot be submitted without a name, contact and consent checkbox. Your consent, given through the separate checkbox, is the legal basis for processing the enquiry. Data is not used for advertising mailings, profiling or automated decisions.
Access and services
Enquiries are stored in a Cloudflare database, with dashboard access restricted to the author. When the Telegram bot is configured, your name, contact, enquiry date and reference are sent to the psychologist’s private chat. The direct link opens Telegram, which has its own terms and privacy policy.
Cloudflare and Telegram operate international infrastructure, so processing may take place outside your country of residence. You can request information about applicable contractual terms and transfer safeguards using the contact above. This form policy does not cover separate therapy records.
Retention
The retention criterion is the time needed to reply and finish arranging a meeting. After that process ends or consent is withdrawn, the enquiry is deleted from the dashboard and separately from Telegram unless another legal obligation requires retention. The author reviews unanswered enquiries monthly and deletes those no longer needed for a reply. This is a manual process; there is no automatic deletion after a fixed number of days.
Your rights
Contact @MaximEMDR to withdraw consent or request access, correction, deletion, restriction or portability where applicable law provides these rights. Withdrawal does not affect the lawfulness of earlier processing. To locate your enquiry, provide the contact you submitted and the approximate date. You can also complain to the competent data protection authority in your jurisdiction.
Technical protection
A short-lived hash of the IP address is used to limit spam; the IP address itself is not stored in the enquiries table. Expired counters are removed during subsequent enquiries. The legal basis for this protection is the legitimate interest in securing the form. Hosting providers may keep technical logs under their own policies. Advertising trackers and third-party analytics are not connected.
